Job summary

FreedomCare is seeking an RN Case Manager to perform in-home assessments, education, and care coordination across CT communities. This one-time contract role offers flexible dates and field visits across West Hartford, New Haven, Bridgeport, Waterbury, and New Britain.

Qualifications

• Must graduate from an accredited nursing program.
• Active and unrestricted RN license in CT.
• Strong clinical assessment, communication, and documentation skills.
• Willingness to travel by car within assigned area.

• Experience in home care or case management is helpful but not required.

Responsibilities

• In-home clinical assessments and initiation of services.
• Develop and update individualized care plans based on patient needs and goals.
• Build ongoing relationships with patients and caregivers through visits.
• Coordinate care with physicians and the wider care team.
• Educate patients and caregivers on care plans and available resources.
• Maintain accurate clinical documentation and ensure regulatory alignment.
