Molina Healthcare, Inc. is seeking a care coordination professional to support integrated member care across the continuum. You will work with a multidisciplinary team to coordinate services, ensure progress toward outcomes, and support cost-effective care.

Responsibilities include assessments, care planning, and collaborative engagement with members, caregivers, and health professionals. Up to 40% local travel may be required based on contracts.

Qualifications

• Must have at least 2 years of healthcare experience, preferably in care management.
• Clinical licensure/certification required only if state contract or license mandates it.
• Valid driver’s license and reliable transportation for travel requirements.
• Knowledge of community resources and local services.
• Able to work proactively with minimal supervision and manage multiple priorities.

Responsabilités

• Complete member assessments per regulated timelines to identify care coordination eligibility.
• Develop and implement care plans with member and care team.
• Conduct telephonic, in-person, or home visits as required.
• Monitor care plan effectiveness and document progress and changes.
• Maintain an ongoing member caseload for outreach and management.
• Promote integration of services across behavioral health, LTSS, and home/community resources.
• Facilitate interdisciplinary care team meetings and collaboration.
• Educate and motivate members using motivational interviewing techniques.
• Identify barriers to care and provide coordination support.
• Collaborate with licensed care managers as needed.
• Travel 25-40% based on state/contract requirements.
