Job summary

CareSource in Alabama seeks a Community Based Care Manager to coordinate care for members across the ICT and OHIO RISE/CME programs. You will serve as the central contact for care coordination, engaging with members, families, providers and community resources.

You will conduct assessments, develop person-centered care plans, and monitor progress using HIPAA-compliant documentation while traveling to member homes and clinics as needed.

Qualifications

• Nursing degree or health care bachelor required.
• RN or clinical licensure in state Ohio required.
• Minimum three years in nursing, social work, counseling, or health care settings.

Responsibilities

• Engage member and natural supports using strength-based, trauma-informed approaches.
• Lead inter-disciplinary care team meetings to address member needs.
• Develop ICP in collaboration with ICT based on member needs and preferences.
• Identify and address barriers to care plan goals.
• Coordinate care transitions and post-discharge planning as needed.
• Document care coordination activities and member response timely.
