Job summary

Qualifications

• Nursing degree or Bachelor's degree in health care.
• Advanced degree related to clinical licensure preferred.
• Case management certification is preferred.

Responsibilities

• Engage member and natural supports using trauma-informed, strength-based assessments.
• Coordinate interdisciplinary care team meetings to review member needs.
• Engage with members in various settings to build professional relationships.
• Develop and regularly update a person-centered care plan with the ICT.
• Identify and manage barriers to achieving care plan goals.
• Implement interventions based on clinical standards and best practices.
• Empower members to manage health through effective care coordination.
• Educate members about treatment options and community resources.
• Document care coordination activities and member responses.
