Job summary

CareSource is seeking a Community Based Care Manager to coordinate care for OhioRISE members within an interdisciplinary team. You will conduct strength-based assessments, develop ICPs, and coordinate with providers and community resources to improve health outcomes.

You will travel regularly to member homes and clinical settings, ensuring timely care planning and documentation while maintaining HIPAA compliance and member advocacy.

Qualifications

• Current, unrestricted clinical license as RN/LISW/LCSW/LCSW/LPCC in Ohio required.
• Case Management Certification preferred.
• Driver’s license, vehicle and verifiable insurance required.

Responsibilities

• Coordinate care with ICT to meet member health and psychosocial needs.
• Develop person-centered care plans with interdisciplinary teams.
• Engage with members in various settings to build professional relationships.
• Identify and address gaps in care and transitions of care.
