Job summary

CareSource in Dayton, OH seeks a Community Based Care Manager to collaborate with the ICT, providers, and community partners to improve quality and address behavioral, physical, and social determinants of health through culturally competent care coordination.

You will assess, plan, and coordinate services, develop person-centered care plans, and facilitate ICT meetings across settings, including member homes and clinics, with travel as needed.

Qualifications

• Nursing degree or health care degree required.
• RN/Clinical Counselor/Social Worker licensure required.
• Minimum 3 years in nursing, social work, counseling or health care.
• Medicaid/Medicare managed care experience preferred.

Responsibilities

• Engage members and natural supports using strength-based and trauma-informed approaches.
• Facilitate ICT meetings and coordinate member care.
• Develop and update person-centered ICPs in collaboration with the ICT.
• Identify barriers and implement evidence-based interventions.
• Coordinate with providers and community resources to ensure seamless care.
