Job summary

CareSource is seeking a Nurse Care Manager to coordinate care across hospital, sub-acute, long-term and home settings. The role includes in-home assessments, planning, facilitation and authorization of covered services to achieve quality, cost-effective outcomes.

The candidate will collaborate with members, families and primary care teams, maintain documentation, and manage complex care plans with a focus on independence and appropriate discharge planning.

Qualifications

• Associates degree in Nursing required; Bachelor's preferred.
• Two years RN experience required; geriatrics or managed long-term care preferred.
• Bilingual abilities preferred; strong communication skills necessary.

Responsibilities

• Provide care coordination including in-home assessment and planning.
• Authorizes and coordinates services across the care continuum with the IDT.
• Document care management activities and recertifications per policy.
