Job summary

CVS Health is seeking a Case Management Coordinator for Ohio, a full-time field-based telework role. You will travel within Northeast Ohio to member homes 50%+ of time, coordinating care, educating members and families, and partnering with MCOs and vendors to deliver integrated behavioral health services.

Required: 2+ years in behavioral health, Bachelor’s or higher in behavioral health or related field, and Ohio delivery system knowledge.

Qualifications

• 2+ years in behavioral health or related human services.
• 2+ years using MS Office Suite (Outlook, Word, Excel, SharePoint).
• 2+ years in children’s mental health or public sector behavioral health.
• 2+ years in areas like family systems, case management or counseling.
• 2+ years experience with Ohio delivery systems and local networks.
• Willing to travel 50%+ and possibly to New Albany for trainings.
• Reliable transportation; mileage reimbursed per policy.
• Willing to work beyond core hours as needed.

Responsibilities

• Evaluate referred members’ needs and eligibility; plan and coordinate care.
• Implement care plans; monitor progress and adjust as needed.
• Collaborate with clinical teams to overcome barriers and coordinate services.
• Engage with members to empower informed health decisions.
• Act as single point of contact for members and document care.
• Maintain compliance with care management and quality standards.
