Job summary

CVS Health is seeking a Field-based Case Management Coordinator in Ohio. The role requires travel within the Western Ohio region to member homes, up to 50% of the time, with Monday–Friday hours and potential after-hours flexibility.

The position emphasizes care coordination, evaluation of member needs, and collaboration with multidisciplinary teams to improve health outcomes for children and families served by OhioRISE.

Qualifications

• 2+ years of experience in behavioral health, social services, or human services.
• 2+ years of experience with MS Office Suite applications (Outlook, Word, Excel, SharePoint).
• 2+ years of experience in children's mental health, child welfare, developmental disabilities, juvenile justice, or public sector human services.

Responsibilities

• Coordinate and implement assigned care plan activities and monitor progress.
• Evaluate member needs/eligibility and determine approaches to care resolution.
• Collaborate with care teams, medical directors, and families to optimize outcomes.
• Provide coaching and support to empower members in healthcare decisions.
