Job summary

CVS Health is seeking a Field-based Case Management Coordinator in Ohio. This full-time telework-friendly position requires travel within the Northeast/Northwest Ohio region up to 50% to member homes and sites.

You will evaluate members, coordinate care plans, and bolster quality of care under the OhioRISE program, with strong collaboration across health care teams. Bachelor level or higher clinician credentials preferred, willingness to work flexible hours.

Qualifications

• Bachelor’s degree or non‑licensed master level clinician required.
• 2+ years of experience in behavioral health, social services, or human services.
• 2+ years of experience with MS Office Suite (Outlook, Word, Excel, SharePoint).
• 2+ years of experience in children’s mental health, child welfare, developmental disabilities, juvenile justice, or public sector human services.
• 2+ years experience in one or more of: family systems, case management, child and family counseling/therapy, child protection, or child development.
• Willing and able to travel within the assigned region up to 50% of the time; Some travel to the New Albany office may be required for trainings/meetings.

Responsibilities

• Evaluate referred member’s needs/eligibility and plan care.
• Coordinate and monitor assigned care plan activities.
• Collaborate with health teams to improve care quality and outcomes.
• Provide coaching, information, and support to empower members in health decisions.
• Serve as a single point of contact for members to remediate care gaps.
