Job summary

CVS Health Corporation is seeking a Case Management Coordinator to support the OhioRISE program, coordinating care for children and families. The role requires field-based travel within Northeast/Northwest Ohio and collaboration with MCOs, vendors, and ODM to optimize outcomes.

Responsibilities include evaluating needs, coordinating care plans, and serving as the main contact for members, with a strong emphasis on quality, documentation, and community-based services.

Qualifications

• Bachelor's degree or non-licensed master level clinician required in behavioral health or related field.
• 2+ years in behavioral health, social or human services with community-based work.
• 2+ years in children’s mental health, child welfare, or public sector health care.

Responsibilities

• Coordinate care for members across child-serving systems with strong documentation.
• Evaluate needs/eligibility and plan care together with multidisciplinary teams.
• Collaborate with medical directors and case managers to resolve barriers to care.
• Act as single point of contact to help members navigate services and resources.
• Adapt to flexible hours and travel as required to support members.
