Job summary

CVS Health Corporation in Ohio seeks a Case Management Coordinator to support the OhioRISE program. This full-time, field-based telework role requires travel up to 50% to member homes and other locations within Northeast/Northwest Ohio.

You will perform care coordination, evaluation of members, and education to empower families. A bachelor's degree or non-licensed master's in behavioral health and 2+ years in child or family services are required.

Qualifications

• Bachelor's degree or non-licensed master level clinician required
• 2+ years of experience in behavioral health, social services, or human services
• 2+ years of experience with personal computers and MS Office applications (Outlook, Word, Excel, SharePoint)
• 2+ years of experience in children's mental health, child welfare, developmental disabilities, juvenile justice, or public-sector human services
• 2+ years experience in family systems, community systems and resources, case management, child counseling/therapy, child protection, or child development
• 2+ years of experience with Ohio delivery systems and local networks

Responsibilities

• Facilitate care coordination and education for members through case management tools
• Evaluate referred members’ needs and eligibility to recommend care plans
• Coordinate and monitor progress of assigned care plan activities
• Collaborate with multidisciplinary teams to overcome barriers and optimize outcomes
• Act as the single point of contact for members to address gaps in care and access
