Job summary

CVS Health Corporation is seeking a Case Management Coordinator in Ohio to support the OhioRISE program. This full-time, field-based telework role requires traveling within Northeast Ohio to member homes and locations, often up to 50% of the time.

Qualifications

• 2+ years of experience in behavioral health or human services
• 2+ years of experience with MS Office Suite
• 2+ years of experience in children's mental health or related field
• 2+ years experience in case management, family counseling, or child development
• Willing and able to travel within Northeast Ohio up to 50% of the time
• Willing and able to work beyond core hours (Mon-Fri 8am-5pm) as needed

Responsibilities

• Evaluate referred members' needs/eligibility and plan care
• Coordinate and monitor care plan activities
• Collaborate with health teams to overcome barriers and improve outcomes
• Provide coaching and support to empower members in healthcare decisions
• Document care processes per regulatory and accreditation guidelines
