Job summary

CVS Health is seeking a Care Management Coordinator for a full-time, field-based telework role in Ohio. The position requires travel within the region up to 50% and residence in central or southeastern Ohio.

You will coordinate care plans, provide education, and work with case managers and Medical Directors to ensure optimal member outcomes. This role emphasizes holistic care, community-based service delivery, and adherence to regulatory guidelines, with a strong focus on coordinating behavioral

Qualifications

• 2+ years of experience in behavioral health, social services, or human services.
• 2+ years of experience with MS Office Suite applications (Outlook, Word, Excel, SharePoint).
• 2+ years of experience in children’s mental health, child welfare, developmental disabilities, juvenile justice, or a public sector human services or behavioral health care field.
• Bachelor’s degree or non-licensed master level clinician.

Responsibilities

• Coordinate and implement assigned care plan activities and monitor care plan progress.
• Provide care coordination, support and education to empower the member to make ongoing medical and healthy lifestyle choices.
• Collaborate with case managers, supervisors, Medical Directors and other health/behavioral health programs to overcome barriers and achieve goals.
• Ensure documentation complies with regulatory and accreditation guidelines and company policies.
