Job summary

Comfort Keepers is seeking a field-based Care coordinator/Community Liaison to build referral networks across Newton to Millis, MA. You will visit hospitals, SNFs, and senior communities, developing partnerships and expanding client opportunities.

The role requires 2–3 years in care coordination or community relations, strong communication, and the ability to manage a territory with independence. Mileage reimbursement and a base salary are offered.

Qualifications

• 2–3 years in care coordination, outside sales, or referral development in healthcare.
• Existing local healthcare and senior care relationships are preferred.
• Knowledge of private-pay home care is highly desirable.

Responsibilities

• Develop and maintain referral relationships with hospitals, facilities, and clinics.
• Conduct in-person visits to introduce services and identify referral opportunities.
• Own territory and build a sustainable referral pipeline.
• Represent Comfort Keepers at community events and network meetings.
• Educate partners and families about in-home care services.
• Maintain records of contacts, referrals, and follow-ups in the system.
• Meet monthly and quarterly business development goals.
