The position focuses on coordinated care for children and families, with emphasis on collaboration across care teams, tracking care plans, and ensuring high-quality, accessible services within Medicaid supports.

Qualifications

• 2+ years in behavioral health, social services, or human services.
• 2+ years using MS Office applications (Outlook, Word, Excel, SharePoint).
• 2+ years in children's mental health, child welfare, or related field.
• Experience with family systems, case management or related areas.

Responsabilités

• Coordinate and inform the case management process to facilitate appropriate healthcare outcomes.
• Collaborate with care managers, supervisors, Medical Directors and other programs to address barriers.
• Coordinate and implement assigned care plan activities and monitor progress.
• Provide coaching and support to empower members in healthcare decisions.

Formation

Bachelor's degree in behavioral health or related field

Non-licensed master level clinician
