Qualifications

• Bachelor's degree or non-licensed master level clinician required.
• 2+ years of experience in behavioral health, social services, or human services.
• 2+ years with MS Office Suite (Outlook, Word, Excel, SharePoint).
• 2+ years in children's mental health, child welfare, or related public sector field.
• 2+ years in family systems, case management, or related areas.
• 2+ years experience with Ohio delivery systems and local networks.
• Willingness to travel 50%+, some trainings at New Albany office; reliable transportation.

Responsibilities

• Evaluate referred member's needs and eligibility and plan care.
• Coordinate and implement assigned care plan activities; monitor progress.
• Collaborate with care teams to overcome barriers and optimize outcomes.
• Escalate quality of care issues and ensure regulatory compliance.
• Provide coaching and information to empower members in healthcare decisions.
• Serve as a single point of contact for members; remediate gaps in care.
