Job summary

Molina Healthcare is seeking a Behavioral Health Care Manager to support care coordination across the continuum. You will work with a multidisciplinary team to ensure member progress toward outcomes and quality, cost-effective care.

The role involves coordinated assessments, care planning, and ongoing monitoring of member needs. The position requires extensive collaboration with clinicians, caregivers, and providers, with travel to member homes or facilities as needed.

Qualifications

• Must have strong verbal and written communication.
• Experience documenting clinical information in electronic records.
• Ability to coordinate with multidisciplinary teams.
• Knowledge of whole-person care principles and discharge planning.

Responsibilities

• Complete behavioral health assessments within timelines.
• Develop and implement care plans with members and care team.
• Conduct visits by phone, in person, or home visits as needed.
• Monitor care plans and update interventions for effectiveness.
• Maintain ongoing member caseload and outreach.
• Promote integration of services across behavioral health and LTSS.
• Facilitate care team meetings and coordinate with ICT.
• Educate and motivate members using motivational interviewing.
• Identify barriers to care and provide assistance.
• Provide consultation to peers when needed.
• Travel 25-40% depending on state requirements.
