Job summary

Molina Healthcare is hiring a care management professional to provide support for care coordination and delivery across the continuum. You will work with a multidisciplinary team to ensure member progress toward outcomes, coordinating with care managers, physicians and support networks.

Qualifications

• At least 2 years experience in health care or care management, or equivalent education/experience.
• Valid driver’s license and reliable transportation for travel requirements.
• Knowledge of community resources and ability to work across settings.

Responsibilities

• Completes member assessments within regulated timelines and identifies care coordination eligibility.
• Develops and implements care plans with member, caregivers and providers.
• Conducts telephonic, in-person or home visits as required.
• Monitors care plans, documents interventions, and suggests changes as needed.
• Maintains ongoing member caseload and performs outreach.
• Promotes integration of behavioral health, LTSS, and community resources.
• Facilitates ICT meetings and collaboration with the care team.
• Uses motivational interviewing to educate and support change.
• Assesses barriers and assists members to address concerns.
• Collaborates with licensed care managers when needed; travel 25–40%.
