Job summary

The role focuses on telephonic and home-visit case management within licensure scope (e.g., Foster Care), coordinating physical and behavioral health and whole-person care for designated populations.

Qualifications

• Requires MS/MA in social work, counseling, or related behavioral health field or nursing degree with relevant clinical experience.
• Active, current and valid license as RN, LCSW, LMHC, LPC, LMFT, or Clinical Psychologist per state law.
• Ability to travel within DSS region and work in field or home visit settings.

Responsibilities

• Conducts assessments to identify individual needs.
• Develops comprehensive care plans addressing identified objectives.
• Supports member access to quality, cost-effective care and adjusts plans as needed.
• Coordinates with internal and external resources for integrated care.
• Incorporates social determinants of health into care planning.
• Engages with state agencies to understand care networks and services.
• Builds partnerships addressing cultural and linguistic needs for better home and community functioning.
• Evaluates health needs and identifies services and resources with members and families.
• Provides patient education, medication reconciliation, and follow-up care arrangements.
