Elevance Health is seeking a Placement Support Specialist (Special Programs Case Manager I) in North Carolina. This field-based role requires licensure and clinical experience, with telephonic assessments and home visits to coordinate care for special programs such as Foster Care.

You will work with internal and external resources to ensure integrated physical and behavioral health, addressing social determinants of health and coordinating with state agencies.

Qualifications

• MSW/MA in social work or nursing degree with 3+ years of relevant clinical experience.
• Active license: RN, LCSW, LMHC, LPC, LMFT, or Clinical Psychologist.
• Experience with complex psychiatric and substance abuse care.

Responsabilités

• Conducts assessments to identify individual needs.
• Develops comprehensive care plan to address objectives and goals identified during assessment.
• Supports member access to appropriate quality and cost-effective care and modifies plan as needed.
• Coordinates with internal and external resources for integrated whole-person care.
• Coordinates social determinants of health to meet member needs.
• Engages with state agencies and networks to align services.
• Builds partnerships with populations and families, addressing cultural and linguistic needs.
• Educates patients, reconciles medications, and arranges follow-up care.
