Qualifications

• MS/MA in social work, counseling, or related behavioral health field or nursing degree with broad clinical experience.
• Active, current and valid license as RN, LCSW, LMHC, LPC, LMFT, or Clinical Psychologist as required by state laws.

Responsabilités

• Conduct assessments to identify individual needs.
• Develop comprehensive care plans addressing objectives and goals from assessment.
• Coordinate with internal/external resources for integrated whole person care.
• Coordinate social determinants of health into care planning.
• Lead specialty projects and initiatives; train and mentor staff.
