Job summary

Qualifications

• MS/MA in social work or related behavioral health field or nursing degree with+ years of clinical experience.
• Active, current, valid license as RN, LCSW, LMHC, LPC, LMFT, or Clinical Psychologist as applicable by state law.

Responsibilities

• Coordinate operations and workflows related to case management in support of specialty programs.
• Coach and mentor staff; lead and participate in specialty projects and initiatives.
• Perform case management telephonically and/or home visits within licensure scope.
• Identify and address social determinants of health in care planning.
• Collaborate with state agencies and community resources to ensure whole-person care.
