Qualifications

• Requires MS/MA in social work or related behavioral health field or nursing degree with 7+ years of clinical social work/counseling experience.
• Requires an active, current and valid license as RN, LCSW, LMHC, LPC, LMFT, or Clinical Psychologist to practice as a health professional within licensure in applicable states.

Responsabilités

• Coordinates operations and workflows for specialty program case management.
• Coaches and mentors staff; leads complex case management activities.
• Performs telephonic and home-visit case management within licensure scope.
• Manages healthcare costs for designated populations via integrated physical and behavioral health care.
• Engages with state agencies and community resources to arrange follow-up care.
• Addresses social determinants of health in care planning.
• Leads specialty projects and process improvements.
• Participates as a leadership team member and supports overall program goals.
