Elevance Health is seeking a Placement Support Specialist to operate primarily in the field across North Carolina, with occasional office attendance. The role focuses on integrated care management, including physical and behavioral health, and coordinating with state agencies and families.

Applicants must hold an active license in nursing or a qualifying behavioral health field and have 3+ years of clinical experience.

Qualifications

• Requires MS/MA in social work, counseling, or related behavioral health field or nursing degree with 3+ years of clinical social work counseling experience.
• Active, current and valid license as RN, LCSW, LMHC, LPC, LMFT, or Clinical Psychologist as applicable by state law.

Responsabilités

• Perform case management telephonically and/or via home visits within licensure scope.
• Manage healthcare costs for designated population via integrated physical and behavioral health case management.
