Qualifications

Requires MS/MA in social work, counseling, or a related behavioral health field or a degree in nursing.
A minimum of 3 years of clinical social work counseling experience with complex psychiatric and substance use treatment; or equivalent.
Active, current, valid license such as an LCSW, LMSW, LPC, LAPC, LMFT, LMHC, or RN in Virginia.

Responsibilities

Conducts assessments to identify member needs.
Develops comprehensive care plans addressing objectives identified during assessment.
Coordinates with internal and external resources for integrated whole-person care.
Coordinates social determinants of health within care planning.
Maintains knowledge of the care system philosophy and community-based services.
