Job summary

Elevance Health is seeking a Placement Support Specialist to perform case management telephonically and via home visits within licensure scope for programs such as Foster Care. The role emphasizes integrated care across physical and behavioral health to manage costs for the designated population.

You will assess needs, develop comprehensive care plans, and coordinate with resources to meet member needs in a whole-person care model. Travel within North Carolina is expected.

Qualifications

• Requires MS/MA in social work or related field.
• Active, current license as RN, LCSW, LMHC, LPC, LMFT, or Clinical Psychologist where allowed.
• Minimum 3 years of clinical experience in social work/counseling with varied psychiatric/substance use experience.

Responsibilities

• Conducts assessments to identify individual needs.
• Develops care plans addressing objectives identified during assessment.
• Supports member access to quality, cost-effective care and adjusts plans as needed.
• Coordinates with internal and external resources for integrated whole-person care.
• Addresses social determinants of health within care planning.
• Collaborates with state agencies and community services.
• Builds partnerships with populations and families, addressing cultural and linguistic needs.
• Provides patient education, medication reconciliation, and follow-up resource assistance.
