Job summary

Molina Healthcare, Inc. seeks an experienced Behavioral Health Care Coordinator to support care management and coordinate services across the care continuum.

You will collaborate with a multidisciplinary team to align member outcomes with quality and cost-effective care strategies. You will assess members, develop care plans with the team, conduct visits (telephonic, in-person, or home), monitor progress, and promote integration of behavioral health, LTSS, and community resources, including

Qualifications

• At least 2 years health care experience, preferably in behavioral health.
• Licensed behavioral health clinician with active license in state of practice.
• Valid driver's license, reliable transportation for travel requirements.
• Experience with severe mental health concerns, substance use, or foster care.
• Knowledge of whole person care, chronic conditions and discharge planning.
• Data entry skills and experience with a clinical platform.
• Excellent verbal and written communication skills.
• Proficiency with Microsoft Office and applicable software.

Responsibilities

• Completes comprehensive behavioral health assessments per timelines and identifies care coordination eligibility.
• Develops and implements care plans with member, family, physician and care team.
• Conducts telephonic, in-person, or home visits as required.
• Monitors care plan effectiveness, documents interventions and adjusts as needed.
• Maintains ongoing member caseload with regular outreach and management.
• Promotes integration of behavioral health, LTSS and community resources.
• Facilitates interdisciplinary care team meetings and collaboration.
• Uses motivational interviewing to educate and motivate change.
• Assesses barriers to care and provides care coordination assistance.
• May provide consultation/recommendations to peers.
• Travel 25-40% depending on state/contractual requirements.
