Qualifikationen

• 2+ years health care experience, preferably behavioral health.
• Active license as a Behavioral Health Clinician (LCSW, LMSW, APSW, CHES, LPC, LPCC, LMFT, PhD or PsyD).
• Valid driver’s license and reliable transportation for local travel.
• Experience with persons with severe mental health concerns and substance use disorders.
• Proficiency with Microsoft Office and clinical software.
• Strong verbal and written communication skills.

Aufgaben

• Support care management/care coordination across multidisciplinary teams.
• Develop and implement care plans with member and care network.
• Conduct telephonic, in-person, or home visits as required.
• Monitor and adjust care plans based on progress and barriers.
• Maintain ongoing member caseload for outreach and management.
• Promote integration of behavioral health, LTSS, and community resources.
• Facilitate care team meetings and ICT collaboration.
• Use motivational interviewing and clinical guidelines to support change.
• Travel 25-40% as required by state/contractual needs.
