Job summary

Pinnacle Fertility, Inc. in New York Brooklyn seeks a Patient Navigator to guide fertility patients through treatment, coordinate care, explain procedures, insurance, and costs, and work with a multidisciplinary team to ensure compassionate, efficient service.

The role is full-time and on-site, with travel between NY locations as needed. The navigator will handle scheduling, EMR documentation, patient education, and after-hours coverage on a rotating schedule.

Qualifications

• Minimum of 2 years in a clinical healthcare setting.
• Strong knowledge of healthcare systems and insurance processes.
• Excellent communication and organizational skills.
• Bilingual skills are a plus.

Responsibilities

• Serve as the primary point of contact for patients, guiding them through treatment plans.
• Coordinate and schedule appointments, tests, and referrals.
• Assist patients with understanding medical care, insurance, and finances.
• Communicate with healthcare providers and administrative teams for care coordination.
• Maintain accurate patient records in the EMR system.
• Provide emotional support to patients and families.
• Participate in team meetings and quality improvement initiatives.
• Educate patients on treatment protocols and medication usage.
• Track progress and follow up on missed appointments.
• Advocate for patient needs and resolve access barriers.
• Assist with after-hours call coverage on a rotating schedule.
