Job summary

Pinnacle Fertility in New York City is seeking a Patient Navigator to guide fertility patients through their care journey. This on-site, full-time role in Brooklyn offers collaboration with a multidisciplinary team to ensure education, scheduling, and compassionate support throughout the treatment process.

Qualifications

• Highschool Diploma required; Bachelor’s degree preferred.

Responsibilities

• Guide patients through treatment plans and education on procedures and follow-up care.
• Coordinate and schedule appointments, tests, and referrals.
• Assist patients with understanding medical care, insurance, and financial resources.
• Maintain accurate patient records in EMR.
• Provide emotional support to patients and families.
• Participate in team meetings and quality improvement initiatives.
• Educate patients on treatment protocols and self-care strategies.
• Track progress and follow up on missed appointments.
