Job summary

Pinnacle Fertility is hiring a Fertility Patient Care Navigator in Westerville, OH. This full-time, onsite role runs Monday through Thursday 8:00 AM–4:00 PM and Fridays 8:00 AM–2:00 PM, with one weekend per month and holiday rotation.

You will guide patients through care plans, educate on procedures and insurance, and coordinate tests and referrals. The ideal candidate has a high school diploma, 2+ years in patient navigation or related fields, and CMA is a plus.

Qualifications

  • Must have a high school diploma; CMA certification is a plus.
  • 2+ years in patient navigation, case management, or a related healthcare role.

Responsibilities

  • Serve as primary contact for patients, guiding treatment plans and educating on procedures and follow-up.
  • Coordinate and schedule appointments, tests, and referrals for seamless care.
  • Assist patients in understanding medical care, insurance coverage, and financial resources.
  • Collaborate with healthcare providers and administrative teams to ensure continuity of care.
  • Maintain accurate EMR records and provide emotional support to patients and families.

About the Role

The Fertility Patient Care Navigator plays an integral role in supporting patients on their journey to parenthood. Working closely with a multidisciplinary team, you’ll guide patients through their care plans, ensuring they receive the support and resources they need every step of the way.

We are seeking a Fertility Patient Care Navigator to join our team at Pinnacle Fertility - Ohio in Westerville, OH. This is a full-time, onsite position, working Monday through Thursday from 8:00 AM to 4:00 PM, and Fridays from 8:00 AM to 2:00 PM. One weekend per month and participation in a holiday rotation is required.

Key Responsibilities
  • Serve as the primary contact for patients, guiding them through treatment plans and educating them on procedures, testing, and follow-up care.
  • Coordinate and schedule patient appointments, tests, and referrals to ensure seamless care.
  • Assist patients in understanding their medical care, insurance coverage, and available financial resources.
  • Collaborate with healthcare providers, clinical staff, and administrative teams to ensure continuity of care.
  • Maintain accurate and up-to-date patient records in the electronic medical record (EMR) system.
  • Provide emotional support to patients and families, addressing concerns and barriers to care.
  • Participate in team meetings, case conferences, and quality improvement initiatives to enhance patient care delivery.
  • Educate patients on treatment protocols, self-care strategies, and medication usage.
  • Track patient progress, follow up on missed appointments, and ensure compliance with treatment plans.
  • Advocate for patients’ needs and assist in resolving barriers to care.
  • Assist with after-hours call coverage on a rotating schedule, ensuring patient needs are addressed appropriately.
  • Perform other duties and projects as assigned.
Position Requirements
  • Education & Experience:
    • High school degree required.
    • Minimum of 2 years in patient navigation, case management, or a related healthcare role.
    • Certified Medical Assistant is a plus.
  • Skills:
    • Strong knowledge of healthcare systems, insurance processes, and patient advocacy.
    • Excellent communication, organizational, and problem-solving skills.
    • Proficiency with electronic medical records (EMR) and healthcare software.
    • Empathy and a patient-focused approach, with the ability to work collaboratively in a multidisciplinary team.
    • Bilingual skills are a plus.
  • Hourly Rate: $22.00 - $26.00 per hour (final compensation based on experience, skills, and qualifications).
