Job summary

Pinnacle Fertility is seeking a Patient Navigator in Chicago, IL to support fertility patients throughout their treatment journey. You will be the primary contact, coordinate appointments, explain procedures and insurance options, and work closely with a multidisciplinary team to ensure excellent patient care.

The ideal candidate has at least 2 years in healthcare customer service, strong communication, and EMR proficiency.

Qualifications

  • Minimum 2 years in customer service or related healthcare role.
  • 1 year fertility experience preferred.
  • Strong knowledge of healthcare systems and insurance processes.
  • Excellent communication and organizational skills.
  • Proficiency with EMR systems.

Responsibilities

  • Serve as primary patient contact guiding treatment plans and education.
  • Coordinate and schedule appointments, tests, referrals.
  • Assist patients with understanding medical care and insurance.
  • Communicate with healthcare providers to ensure care coordination.
  • Maintain accurate EMR documentation.
  • Provide emotional support and resources to patients and families.
  • Participate in team meetings and quality improvement initiatives.
  • Educate patients on treatment protocols and self-care.
  • Track progress and follow up on missed appointments.
  • Advocate for patient needs and resolve barriers.
  • Assist with after-hours call coverage on rotating schedule.
  • Other duties as assigned.

About the Role

The Patient Navigator is an ideal opportunity for those seeking to be an integral part of a fertility patient’s journey toward becoming a parent. The Patient Navigator position at Pinnacle will allow you to engage with the patient throughout their journey while working closely with a multidisciplinary team to provide excellent patient care.

We are seeking a Patient Navigator to join our dedicated team at Pinnacle Fertility - Illinois in Chicago, IL. This is a full-time, on-site position, working Monday - Friday, 8:30 AM - 5:00 PM.

Key Responsibilities
  • Serve as the primary point of contact for patients, guiding them through their treatment plan and providing education on procedures, testing, and follow-up care.
  • Coordinate and schedule patient appointments, tests, and referrals to ensure a smooth continuum of care.
  • Assist patients with understanding their medical care, insurance coverage, and available financial resources.
  • Communicate with healthcare providers, clinical staff, and administrative teams to ensure seamless patient care coordination.
  • Maintain accurate and up-to-date patient records, including documentation of interactions and care plans in the electronic medical record (EMR) system.
  • Provide emotional support and resources to patients and their families, addressing any concerns or barriers to care.
  • Participate in team meetings, case conferences, and quality improvement initiatives to enhance patient care delivery.
  • Educate patients on treatment protocols, self-care strategies, and medication usage.
  • Track patient progress, follow up on missed appointments, and ensure compliance with treatment plans.
  • Advocate for patients’ needs and help resolve any issues or barriers to care.
  • Assist with after-hours call coverage on a rotating schedule, ensuring patient needs are addressed appropriately.

Position Requirements
  • Education: High School Diploma required. Bachelor’s degree (preferred)
  • Experience:
    • Minimum of 2 years of experience in customer service, patient navigation, case management, or a related healthcare role.
    • 1 year of fertility experience preferred.
  • Skills:
    • Strong knowledge of healthcare systems, insurance processes, and patient advocacy.
    • Excellent communication, organizational, and problem-solving skills.
    • Proficiency in using electronic medical records (EMR) and other healthcare software.
    • Bilingual skills are a plus.
COMPENSATION & BENEFITS
  • Hourly Rate: $24.00 - $26.00 (Final offers based on experience, skills, and qualifications.)
