Job summary

Pinnacle Fertility in Arlington, VA seeks a Patient Navigator to guide fertility patients through their treatment journey, coordinating care and educating patients while working with a multidisciplinary team to ensure excellent patient care.

The role is full-time and on-site, requiring a minimum of 2 years in a clinical healthcare setting, strong EMR and insurance knowledge, and exemplary communication. Bilingual skills are a plus. After-hours coverage may be required.

Qualifications

  • Minimum 2 years of experience in a clinical healthcare setting, including patient care coordination, patient navigation, or case management.
  • Strong knowledge of healthcare systems, insurance processes, and patient advocacy.
  • Excellent communication, organizational, and problem-solving skills.

Responsibilities

  • Serve as primary point of contact for patients and educate on procedures.
  • Coordinate and schedule patient appointments, tests, and referrals.
  • Assist patients with understanding their medical care, insurance coverage, and available financial resources.
  • Communicate with healthcare providers, clinical staff, and administrative teams to ensure seamless patient care coordination.
  • Maintain accurate and up-to-date patient records in the EMR system.
  • Provide emotional support and resources to patients and families.
  • Participate in team meetings, case conferences, and quality improvement initiatives.
  • Educate patients on treatment protocols, self-care strategies, and medication usage.
  • Track patient progress, follow up on missed appointments, and ensure compliance with treatment plans.
  • Advocate for patients’ needs and help resolve barriers to care.
  • Assist with after-hours call coverage on a rotating schedule.
  • Other duties and projects as assigned

We are seeking a Patient Navigator to join our dedicated team at Pinnacle Fertility - Virginia located in Arlington, VA. This is a full-time, on-site position.

Key Responsibilities
  • Serve as the primary point of contact for patients, guiding them through their treatment plan and providing education on procedures, testing, and follow-up care.
  • Coordinate and schedule patient appointments, tests, and referrals to ensure a smooth continuum of care.
  • Assist patients with understanding their medical care, insurance coverage, and available financial resources.
  • Communicate with healthcare providers, clinical staff, and administrative teams to ensure seamless patient care coordination.
  • Maintain accurate and up-to-date patient records, including documentation of interactions and care plans in the electronic medical record (EMR) system.
  • Provide emotional support and resources to patients and their families, addressing any concerns or barriers to care.
  • Participate in team meetings, case conferences, and quality improvement initiatives to enhance patient care delivery.
  • Educate patients on treatment protocols, self-care strategies, and medication usage.
  • Track patient progress, follow up on missed appointments, and ensure compliance with treatment plans.
