Job summary

Rock Valley Community Programs, Inc. is seeking a Federal Case Manager to support residents in a federal residential re-entry program. You will manage caseloads, conduct meetings, coordinate with contractors, and ensure compliance with FBOP SOW and agency policies.

The role involves planning, monitoring progress, and facilitating access to housing, employment, and community resources, with occasional on-site facility operations and trainings.

Qualifications

• Degree and/or equivalent experience in Human Services, Criminal Justice, or Counseling.
• Strong written and verbal communication skills.
• Ability to work collaboratively within a multidisciplinary environment.
• Must be able to sit, stand and walk for extended periods.

Responsibilities

• Develop and maintain individualized re-entry plans addressing housing, employment, financial stability, health needs, education, and reintegration.
• Conduct regular sessions to monitor progress, address barriers, and support release planning.
• Coordinate services with USPOs, therapists, placement specialists, and community providers.
• Complete documentation including intake, program plans, progress notes, and discharge summaries.
