Job summary

Rock Valley Community Programs is seeking a Federal Case Manager to provide case management, re-entry planning, and care coordination for residents in a federal residential program.

Mon-Fri schedule with duties including planning, sessions, housing and employment assistance, and collaboration with USPOs and therapists. Background check required, health/dental/vision benefits offered after hire.

Qualifications

• Degree and/or equivalent experience in Human Services, Criminal Justice, or Counseling.
• Strong written and verbal communication skills.
• Ability to work collaboratively within a multidisciplinary environment.
• Ability to manage high volume client interactions in a group setting.
• Proficiency in Microsoft Office.
• Must be able to sit, stand and walk for extended periods.
• Occasionally lift/move up to 25 pounds.
• Valid driver’s license and clean driving record.
• Must complete TB test and drug screening (on-site).
• Completion of CPR, First Aid, CBRF Fire Safety, and Choking training within 90 days.
• Employment is contingent upon passing state and federal background check.
• Health/Dental/Vision Insurance available after date of hire.

Responsibilities

• Develop and maintain individualized re-entry plans addressing housing, employment, finances, health, and education.
• Conduct regular individual sessions to monitor progress and support release planning.
• Assist residents with securing housing, employment, education, and community resources.
• Monitor resident behavior, participation, and compliance with program requirements.
• Review and approve resident passes and monitor home confinement and GPS compliance.
• Complete employment verification and employer contacts as required by FBOP SOW.
• Coordinate services with USPOs, Therapists, and community providers.
• Provide referrals and facilitate access to services for resident success.
• Maintain professional relationships with residents, contractors, and staff.
• Complete intake paperwork, program plans, progress reports, and discharge summaries.
• Ensure timely, accurate reporting to meet FBOP SOW and agency standards.
• Audit resident files and present updates at PRT meetings.

Rock Valley Community Programs provides transitional and supportive housing, addiction and mental health treatment services to adults in the Rock County area. We provide these services to homeless male veterans, clients connected to the criminal justice system, and Rock County individuals needing assistance with addiction and/or mental health treatment.

Position

The Federal Case Manager provides case management, re-entry planning, and care coordination services for residents participating in a federal residential re-entry program. Working under the supervision of the Federal Program Director, this position supports continuity of care and successful community reintegration through individualized programming and service coordination.

Key Responsibilities

• Develop and maintain individualized re-entry plans that address housing, employment, financial stability, health needs, education, and successful community reintegration.
• Conduct regular individual sessions to monitor progress, address barriers, reinforce accountability, and support release planning.
• Assist residents with securing housing, employment, educational opportunities, and community resources.
• Monitor resident behavior, program participation, and compliance with RVCP and Federal Bureau of Prisons (FBOP) requirements.
• Review and approve resident passes, monitor home confinement and GPS compliance, and ensure required registrations and reporting obligations are met.
• Complete employment verification and ongoing employer contacts as required by the FBOP Statement of Work (SOW).
• Coordinate services with USPOs, Therapists, Employment Placement Specialists, community providers, and other stakeholders to ensure continuity of care.
• Provide referrals and facilitate access to services that support resident success and re-entry goals.
• Maintain professional relationships with residents, contractors, community partners, and agency staff.
• Complete and maintain all required documentation, including intake paperwork, Individualized Program Plans, progress reports, case notes, incident reports, home confinement paperwork, exit plans, and discharge summaries.
• Ensure all reporting is completed accurately and within required timelines to meet FBOP SOW and agency standards.
• Audit resident files for compliance, present resident updates at Program Review Team (PRT) meetings, attend required meetings and trainings, and assist with facility operations as needed, including facility searches and computer lab monitoring.
