Health Connect America is seeking a Family Support Specialist to deliver best-practice clinical services for children, families, and adults in our multi-state footprint. You will help develop treatment plans, provide individual, group, and family interventions, and ensure all documentation meets COA and regulatory standards.

Qualifications

• Bachelor’s degree in human services or related field, or BA/BS with 3+ years of relevant experience.
• 1 year of experience working with children (including internships or volunteer experience) preferred.
• Strong communication and collaboration with treatment teams and clients.

Responsibilities

• Communicate with clients and families to answer questions and address concerns.
• Develop treatment plans with input from the treatment team and Clinical Supervisor.
• Provide individual, group, and family therapy as outlined in the treatment plan.
• Implement approved therapeutic interventions with clients and families.
• Complete client documentation within established timeframes.
• Attend weekly Treatment Team meetings and participate actively.
• Link clients and families with services identified in the plan.
• Serve as liaison with other professionals and agencies.
