Health Connect America, Inc. is seeking a Family Support Specialist to provide best practice clinical services for children, families, and adults while ensuring documentation meets COA, payer, and state standards.

The role involves developing treatment plans, delivering individual/group/family therapies, and coordinating with the treatment team to connect clients with needed services. The position includes full-time employment with benefits, opportunity for professional development, and on-call

Qualifications

• Bachelor's degree in a human services discipline (Social Work, Psychology, Sociology, Counseling, or Criminal Justice) with 1 year of experience with children preferred, OR a BA/BS with 3 years of relevant experience.

Responsibilities

• Communicate with customers, colleagues, and other individuals to answer questions or concerns.
• Develop treatment plans with input from the treatment team and Clinical Supervisor.
• Provide individual, group, and family therapeutic interventions per the treatment plan.
• Implement approved therapeutic interventions with clients and families.
• Complete all client documentation within established time frames.
• Attend and participate in weekly Treatment Team meetings.
• Link clients and families with services identified in the plan.

Education
