BCFS HHS is seeking a Family Support Specialist to provide case management and crisis intervention for youth aged 6-18 and their families across Texas. You will conduct individual and family sessions, coordinate referrals, and document services while ensuring compliance with state and federal guidelines.

The role requires driving to client homes, scheduling flexibility including evenings/weekends, and maintaining confidentiality and professional standards.

Qualifikationen

• High school diploma or equivalent required.
• 4 years of direct-services experience with at-risk youth and families required.
• Bachelor’s degree may substitute for work experience.

Aufgaben

• Provide individual and family sessions using Motivational Interviewing.
• Facilitate the Strengthening Families Program (SFP) virtually, in-office, or at other locations.
• Coordinate referrals, service planning and documentation for caseloads.
• Respond to crises promptly and professionally in person, at home, or by phone.
• Develop and monitor treatment plans to increase strengths and reduce home conflict.
• Assist in emergency shelter placements and respite care when appropriate.
• Report suspected abuse or neglect to supervisor and agencies.
• Attend staff meetings and training; participate in UPCA events.
• Provide regular supervision with supervisor.
• Travel to client homes and community partners as needed.
• Meet deadlines and maintain compliance with grant requirements.
