Job summary

Youth Villages is seeking a Family Support Specialist in Tennessee to provide family-centered, strength-based services under the Family Support Services program. You will handle case management, crisis support, and care coordination for youth from birth to age 18, partnering with DCS and other agencies to plan services and improve outcomes.

The role requires strong organizational skills, the ability to manage multiple priorities, and a valid driver's license for community-based work.

Qualifications

• Bachelor's degree in social services field (required)
• Master’s degree in a social services discipline (preferred)
• Experience with the Foster Care System and/or DCS (preferred)
• Clinical experience and/or case management (preferred)
• Strong organizational skills and attention to detail
• Excellent written, verbal, and oral skills
• Ability to manage multiple priorities simultaneously

Responsibilities

• Carries a caseload of approximately 20 cases
• Conducts home and site visits at least 2 times per month to assess safety concerns
• Provides case management and care coordination services to youth and families
• Facilitates the youth’s treatment progress in the least restrictive safe setting
• Ensures documentation in an electronic medical record system (EMR) is accurate and timely
• Attends meetings as an advocate for youth and family (team, court, school)
• Performs other duties as assigned
