Job summary

BCFS Health and Human Services seeks a Family Support Specialist to provide case management and crisis intervention for at-risk youth and families. You will conduct individual and family sessions, coordinate referrals, and document services in compliance with state and federal standards.

Responsibilities include developing action plans, assisting with shelter placements, maintaining confidentiality, and traveling to client homes and partner sites as needed.

Qualifications

• Bachelor's degree in Social Work, Sociology, Psychology, Counseling or Criminal Justice.
• Or four years of work experience providing direct services to at-risk youth and families.
• Experience serving vulnerable populations.

Responsibilities

• Provide individual and family sessions using MI and SFP.
• Coordinate referrals, service planning and document services for caseload.
• Respond to crises with independent judgment during office visits, home visits, or calls.
• Develop and monitor action plans to increase strengths and reduce home conflict.
• Assist in emergency shelter placements or respite care when appropriate.
• Maintain confidentiality and safety protocols.
• Attend staff meetings, trainings and UCAP events; participate in supervision.
• Travel to client homes and partner sites as needed.
• Meet deadlines with supervisor and state/federal partners.
