Job summary

Health Connect America in Tennessee seeks a Family Centered Treatment Specialist to deliver in-home and community-based psychotherapeutic services to children, adolescents, and families using the FCT model. The role emphasizes family-centered planning, ongoing assessments, and collaboration with the Treatment Team to ensure coordinated care.

The successful candidate will hold a relevant degree, have or pursue FCT certification, and participate in after-hours on-call coverage with a flexible

Qualifications

• Bachelor’s degree in human services or related field; two years’ experience with youth or families.
• FCT Certification preferred; if not certified, must pursue certification within one year of employment.
• Experience delivering Family Centered Treatment in home/community settings.

Responsibilities

• Provide Family Centered Treatment (FCT) services per model fidelity.
• Maintain FCT documentation and service plans.
• Coordinate with Treatment Team for service planning and discharge.
• Deliver psychoeducation and family interventions.
• Ensure flexible scheduling including 24/7 on-call coverage as needed.
• Engage with community resources and referrals.
• Complete clinical records within required timelines.
