Child and Family Focus is seeking a Family Support Partner in Bucks County, PA to work with families and youth through the High-Fidelity Family Teams process. The role focuses on empowering families, coordinating natural and community supports, and guiding individualized plans developed with the family.

Responsibilities include facilitating meetings, modeling trauma-informed practices, and driving engagement while traveling across Bucks County.

Qualifications

• Be the parent of a child with complex emotional and behavioral needs.
• High school diploma or GED.
• Able to attend evening appointments (5-7:30pm) 2-3 days per week.

Responsibilities

• Provide family support and guidance through the High-Fidelity Family Teams process.
• Facilitate meetings and assist in completing individualized plans.
• Help build a team of natural, community, and systems supports for the family.
