Job summary

Community Behavioral Health in Philadelphia seeks a Family Support Partner to assist caregivers of youth navigating child-serving systems. This role draws on lived experience to provide empathetic support within the Clinical Management Department.

You will conduct outreach, coordinate with DHS and school partners, attend inter-agency meetings, and help families access resources addressing social determinants of health. Travel across Philadelphia is required.

Qualifications

• Education: High School Diploma or GED preferred.
• License/Certification: PA Child Abuse Clearance required; Family Peer Support Specialist Certification within 6 months of hire.
• Experience: 1-2 years assisting families navigating child-serving systems.

Responsibilities

• Conducts outreach and builds rapport with family members of youth referred to child services.
• Assists families in completing family satisfaction surveys.
• Coordinates with cross-system agencies (DHS, CUA, Probation, social workers, schools).
• Attends provider meetings and weekly rounds.
• Maintains outreach activity records and supports care coordination.
