Qualifications

• Bachelor’s degree or equivalent in a related field.
• Experience in social services, community outreach, family support, or related work preferred.
• Commitment to strengths-based, family-focused, trauma-informed service planning.
• Ability to protect confidentiality and work with diverse communities.

Responsibilities

• Build partnerships with community organizations, schools, health providers, and public agencies.
• Coordinate programs such as after-school activities, wellness events, education workshops, and job readiness activities.
• Engage residents through outreach and group activities to identify needs and connect families to resources.
• Provide direct resident support, including assessments, service planning, referrals, and follow-up.
• Maintain records of resident needs, referrals, progress, and outcomes in required systems.
