Job summary

Urban Strategies in Saint Louis, MO, is seeking a Family Support Specialist II to provide case management and coordination to promote economic self-sufficiency for families. You will work with a team of social workers and relocation/employment staff to implement plans under the Promise Neighborhood Improvement grant.

Qualifications

• Bachelor's degree in a relevant field is required.
• Minimum of three years of direct case management with low-income households.

• Availability for evening/weekend functions and valid driver's license.

Responsibilities

• Conduct intake and assessments for enrollment into the case management program.
• Perform home and community visits to collect information from residents.
• Develop, implement and monitor Development Plans with a multidisciplinary team.
• Assist residents navigating social services and public agencies.
• Identify gaps in services and coordinate service delivery for assigned caseload.
• Maintain privacy and document progress in LEARN real-time.
