Qualifications

  • Ability to understand and apply place-based, resident-centered program models and work collaboratively with families and community partners.

Responsibilities

  • Build partnerships with community-based organizations, schools, health providers, workforce programs, and public agencies that respond to resident-identified needs.
  • Coordinate programs such as after-school activities, health and wellness events, education workshops, job readiness activities, onsite classes, and other family-centered services.
  • Engage residents through outreach, one-on-one support, group activities, and community meetings to understand priorities, identify barriers, and connect families to appropriate resources.
  • Provide direct resident support, including conducting assessments, developing family-centered service plans, making referrals, coordinating follow-up, and completing quarterly check-ins for residents with identified goals.
  • Identify resident and family needs related to housing stability, health, education, employment, safety, and overall well-being, and locate community resources to address those needs.
  • Collaborate with property management, resident leadership, schools, service providers, and community partners on neighborhood safety, resident engagement, and family support initiatives.
  • Assist with program development, resource coordination, grant-related activities, and local site initiatives that strengthen services for residents and families.
  • Maintain accurate records and document resident needs, referrals, progress, program participation, and outcomes in required systems, including LEARN.

JOB SUMMARY

The Family Support Specialist (FSS) will serve as a trusted resource and connector for residents, families, and community partners in a defined community or housing development. This role is ideal for candidates with experience in social work, community outreach, case management, education, health promotion, resident services, or community-based programming. The Specialist will work directly with residents to identify strengths, address barriers, connect families to services, and support housing stability, wellness, education, employment, and overall family success. The FSS will also build relationships with schools, health providers, nonprofit organizations, public agencies, and resident leaders to develop responsive programs that reflect community priorities and promote long-term positive outcomes.

Direct Service Delivery and Community Engagement Responsibilities

QUALIFICATIONS
  • Bachelor's degree in social work, sociology, psychology, human services, public health, education, community development, or a related field. Equivalent relevant experience may be considered.
  • Experience in social services, community outreach, family support, resident services, education programs, health promotion, case management, or nonprofit/community-based work preferred.
  • Commitment to strengths-based, family-focused, trauma-informed, and culturally responsive service planning.
  • Ability to understand and apply place-based, resident-centered program models and work collaboratively with families and community partners.
  • Ability and commitment to handle confidential and privileged information in a professional manner.
  • Ability to work effectively as part of a team and maintain positive internal and external relationships.
  • Possession of a valid driver’s license and availability of automobile for work use.
  • Strong verbal and written communication skills.
  • Proficiency in Microsoft Outlook, Word, Excel, and commonly used data or case management systems.
