Job summary

Urban Strategies, Inc. is seeking a Family Support Specialist II in Saint Louis, Missouri. The role focuses on comprehensive case management, referrals, and coordination with community services to promote economic self-sufficiency for families, seniors, and the disabled.

You will work with a team of social workers and relocation and employment specialists on the Promise Neighborhood Improvement Grant. Occasional nights/weekends and travel may be required.

Qualifications

• Bachelor's Degree in Social Work, Sociology, Psychology or other relevant field.
• Minimum three years of direct case management experience with low-income households.
• Ability to handle confidential information and work in a team.
• Proficiency with MS Outlook, Word and Excel.

Responsibilities

• Conduct intake and assessments for enrollment into the case management program.
• Perform home and community visits to collect information.
• Develop, implement, monitor, and modify Development Plans with a multidisciplinary team.
• Navigate social service agencies and advocate for residents as needed.
• Maintain privacy and confidentiality of all household information.
