The position collaborates with a team of social workers, relocation and employment specialists, and other professionals to support families and promote self-sufficiency. The role requires assessing residents, developing development plans, and guiding services while maintaining privacy.

Qualifications

• Bachelor's degree in social work, sociology, psychology or related field.
• At least three years of direct case management or related service experience with low-income households.
• Commitment to strengths-based, family-focused service planning.
• Ability to handle privileged information professionally and confidentially.
• Strong communication, organizational and time-management skills.

Responsibilities

• Conduct intake and assessments for residents enrolling in the case management program.
• Conduct home, office, and community visits to collect information from residents.
• Develop, implement, and monitor Development Plans with a multidisciplinary team.
• Assist residents in navigating social service agencies and programs.
• Identify gaps in services, advocate for residents, and coordinate service delivery.
• Enter and track resident data in LEARN and maintain privacy of information.
