Job summary

High Point & Affiliated Organizations is seeking a Family Support Navigator for the Youth Point program in Brockton, MA. The role will support open access screening for mental health and substance use services, assist families with intake, and coordinate care transitions within the CBHC framework.

The ideal candidate has caregiver experience, a psychology-related degree (preferred) or equivalent, and strong interpersonal skills. Bi-lingual applicants are encouraged.

Qualifications

• Preference for someone with lived caregiver experience
• 2 years of behavioral health experience
• Bi-lingual/multi-lingual strongly encouraged
• Excellent interpersonal and communication skills
• Knowledge of basic computer skills and EMR systems

Responsibilities

• Support open access model for screening and assessments for mental health and substance use services
• Assist patients and families with intake paperwork and accessing CBHC services
• Coordinate patient transportation and discharge planning activities
• Engage in community outreach to support access to treatment
• Maintain contacts with other providers and care team as needed
• Educate caregivers on accessing community resources and navigating services
